Living Will Form
Say how you want to be treated if you cannot say it.
A living will is a written statement of your wishes for medical treatment if you become unable to communicate them. This free form gives you the structure to record your preferences, name the person to speak for you, and sign it properly.
What this form is for
A living will (an advance healthcare directive) states the treatment you do and do not want when you cannot decide for yourself. It takes effect only if you become unable to communicate your wishes.
It works alongside a healthcare power of attorney, which names the person to make decisions. Together they are the two parts of an advance directive.
Key parts
- Your identity and the state where it applies
- Your treatment preferences — what you do and do not want
- Who speaks for you (the healthcare agent)
- When the document takes effect
- The signature and witness or notarization requirement
Fill in your details
These forms are starting structures for you to fill in yourself, not legal advice. Anything that will have legal effect, or that varies by jurisdiction, should be reviewed by someone qualified in the relevant place.
